sizing off center

A customer sent me these photos of his 35 Remington bullet. Obviously it was sized off center, to the point that one side of the front band is completely gone.

I believe he was using an RCBS lubrisizer but otherwise I don't know the deets. The most common cause is lack of a generous taper at the entrance of the die. Another cause could be the nose punch or the ram that holds the nose punch not being concentric with the die. My bet is on a lack of tapered entrance at the die, it's a very common problem.
